Please submit updated immunization documentation to the Health Center when changes occur. Per Arizona State Statute, "a pupil shall not be allowed to attend school without submitting documentary proof to the school administration unless the pupil is exempted from immunizations."
Maricopa County has several clinics that offer free immunizations. In addition to the vaccines required for all K-12 students, 1 dose of Tdap and 1 dose of quadrivalent meningococcal vaccine are required when a student turns 11 years old, regardless of grade. The Arizona Department of Health recommends no more than 15 days after their 11th birthday.

Note: The immunization clinics tend to be slower during the month of May and June and are the busiest as we approach the start of the school year. In order to avoid delaying a child's entry into school, we encourage families to contact their provider or schedule an appointment at one of the MCDPH Immunization Clinics as soon as possible.
